When You Meet with Us

We will discuss your goals, review your documents up-to-date and develop a plan to pay for your loved one’s care, including exploring Medicaid and VA benefits.

We can also pair you with an Elder Care Coordinator who is a nurse or social worker who will:

  • Evaluate your loved one’s medical needs, capacity, and limitations
  • Draw on years of experience to help achieve your goals such as staying at home.
  • Find a trusted contractor who can modify the home to make it safe.
  • Arrange for a reliable home care agency to help with everyday tasks and medication management to ensure your loved one is getting the right medical attention.
  • Set up your loved one with a respected geriatric doctor.

We work to provide you with the peace of mind you need. Our Life Care Planning Team is in place to help you choose the right facility, advocate for care, execute your plan to pay for care and answer all of your questions along the way.
