Celebrating National Social Work Month: The Heart of Elder Care Coordination

March is National Social Work Month – a time to recognize the dedicated professionals who stand in the gap for families navigating life’s most difficult transitions. As I often tell clients, central to what we do at The Law Practice of Dennison Keller, LLC, are our Elder Care Coordinators (ECCs). Whether trained professionally as social workers or nurses, the ECCs are the advocates and problem-solvers who ensure our clients are safe, respected, and well cared for.

To celebrate National Social Work Month, this month we’re highlighting a few success stories from our practice that showcase the life-changing impact of expert Elder Care Coordination.

Advocacy That Changes Lives

Our Elder Care Coordinators are often the “eyes and ears” for families. In one instance, an ECC was visiting a client in a nursing home and noticed a chronic wound on the resident’s tailbone caused by sitting for long hours in a wheelchair. The ECC didn’t just note the issue – she took immediate action, collaborating with the nursing staff to find and use a specialized cushion designed to prevent and treat ulcers. The cushion relieves pressure and enhances blood flow. Next, our ECC offered other suggestions to facilitate healing. In a few weeks, the sore had cleared, directly improving our client’s health.

In another powerful example of advocacy, a client residing in a nursing home was struggling because of a language barrier and health complications involving tube feeding. Specifically, she was frequently aspirating her nutritional supplement. Our Elder Care Coordinator stepped in to bridge the communication gap and suggested specific adjustments, including elevating the client’s bed to prevent aspiration.

When families have reported care concerns that could pose a risk to our clients with diminished capacity, I have seen our ECCs conduct a series of unannounced site visits at senior living communities. This ensures not just the client’s safety and well-being, but also provides peace of mind to their loved ones that the community is upholding promised accountability. These actions aren’t just medical notes; they are regular acts of dignity that elevate our clients’ quality of life.

Finding the Way Back Home or Staying Happy and Healthy at Home

For many seniors, the ultimate goal is to return to the comfort of their own home. We worked with a client who had been in a nursing home for two years following a fall. While the family desperately wanted her home, the logistics felt impossible. Through persistent visits and coordination, our Elder Care Coordinator helped pave the way for her to safely return to her own residence, proving that with the right support, the “impossible” becomes possible.

For clients whose goal is to stay home as long as it is safely possible, our ECCs work hard to stay up to date on the latest technology and resources to help them achieve that goal. Whether it’s referring to a tried-and-true community resource or just researching and researching and researching until we find a new provider to help solve a client’s problem, our ECCs provide meaningful action steps to families so their loved one can stay healthy at home longer.

Supporting the Caregivers

Speaking of families, Elder Care Coordination can sometimes be as much about the family as it is about the senior client themselves. We often see a spouse at her “wits’ end” trying to care for her husband. In one case, the husband’s severe mobility issues and inconsistent service from home health agencies had left her exhausted. Our team stepped in to stabilize the situation, providing the professional guidance and resource coordination she needed to secure stable caregivers so she could breathe again. Sometimes, just being a wife and partner, rather than an “always on” caregiver, is the most meaningful help we can provide.

One of my proudest moments working for the firm took place when we were all attending a conference three states away. Our ECC was on the phone with me, helping an overwhelmed husband through the safe discharge plan for his wife, who suffered from dementia. Seeing her compassion for him, as well as her knowledge of how best to support the wife’s needs as she left the hospital, affirms to this day my belief that social workers are the MacGyvers of our society. Give them a little bit of time, a paperclip, and a stable internet connection, and they will diffuse the most difficult “bomb” of family care crises!

More Than Just a Plan

Whether it’s ensuring “No Pet is Left Behind” during a sudden transition or navigating the “maze” of Medicare and Medicaid benefits, our Elder Care Coordinators bring a personal touch to a complex system. They are the ones holding hands, watching the details of care, and, as one family put it, acting as the “heart” that continues to give honor and love to our clients. This month, we invite you to join us in thanking the social workers and care coordinators who turn a “dark tunnel” of uncertainty into a path toward peace and comfort.
